To ensure that cost will not be a barrier to our community we offer financial assistance and help patients apply for programs that they may be eligible for.

Financial Assistance

• Financial Assistance Program - Frisbie Memorial Hospital offers an application process based on income, geographical area and other guidelines. In applying for financial assistance our financial counselors will assess your eligibility for federal or state programs as well. If it is determine that you may be eligible for one of these programs, we will assist you in the application process.

• New Hampshire Health Access Network- Frisbie Memorial Hospital participates in New Hampshire Health Access Network which provides assistance to the uninsured and the underinsured with medical expenses. This is an application process where eligibility is determined by guidelines set by the Foundation of Healthy Communities. • Self-Pay Discount - After receiving your first bill you may take a 20% discount if paid in full and if paid within 20 days. A 15% discount can be taken if paid in full within 45 days.

• New Hampshire Medicaid - To apply or to get more information please call the Department of Health and Human Services at 603-332-9120 or 1-800-862-5300. You may also call our financial officer's office and they can assist you in the application process.

• Maine Medicaid - To apply or get more information please call the MaineCare Department of Health and Human Services at 1-800-482-0790 or call our financial counselor's office and they can assist you in the application process.

• New Hampshire Healthy Kids - Provides health and dental insurance for children under the age of 19. The health insurance may also extend to women that are pregnant. To apply or get more information please call 1-800-852-3345 or call our financial counselor's office who can assist you with the process.

• Making Payments - We know that health care costs can come up unexpectedly. Whether you are insured or uninsured, whether or not you qualify for financial assistance, we understand that you may have difficulty paying your healthcare costs in full at the time of service or in a lump sum. WE are happy to work with any patient to set up a payment plan.

How To Apply For Financial Assistance

To apply for financial assistance or to get more information on programs that you may be eligible for please call our financial counselors. The financial counselors are available to meet directly with you or mail applications to your home. The financial counselors are available Monday through Friday between 8:00 a.m. and 4:30 p.m.

Notification

Frisbie Memorial Hospital will notify patients in writing that have applied for financial assistance whether they are approved or denied. A denial letter is sent with an explanation of denial. Included with the denial letter is an appeal letter that can be completed if the patient feels necessary. If the application is approved, an approval letter is sent with a determination date.

For New Hampshire Health Access Network, the process is the same except that a card will be included with the approval letter. This card will have the issuing organization, the level of discount and the effective dates.

Please note that financial assistance is valid for six months and then a new application must be completed after that time.
