Medication Assistance Program

Our Medication Assistance Program (MAP) offers free prescription drugs to those who have a chronic health condition and whose primary care provider is part of the Frisbie Memorial Hospital Physician Practice Services.  Patients must also meet certain financial criteria to be eligible for the Medication Assistance Program.

Service area includes:

  • Barrington
  • East Rochester
  • Farmington
  • Gonic
  • Middleton
  • Milton
  • New Durham
  • Rochester
  • Sanbornville
  • Somersworth
  • Strafford
  • Berwick and Lebanon, Maine

A program representative will review the patient's application to determine eligibility. Qualifying applicants' information will be submitted to their primary care provider for signature. Signed applications will then be forwarded to those pharmaceutical companies that offer free prescription programs for final review.

The pharmaceutical company will determine if applicants meet their criteria to participate in a prescription assistance program. If your application is accepted, medication will be mailed to your primary care provider's office on your behalf - a process that may take six to eight weeks from the time the application is submitted.
