Care Management

Our Care Management Team is comprised of nurses, social workers, discharge planners, and health insurance specialists cross-trained in providing care and advocacy to help meet each inpatient's individual clinical and emotional needs. Specially trained care managers work to help patients and families cope with illness and to develop a plan for a safe discharge home or to another facility. We make patient referrals to outside agencies such as rehabilitation facilities and visiting nurse services. They can also provide information regarding financial assistance programs.

Medication Assistance Program

Our Medication Assistance Program (MAP) offers free prescription drugs to those who have a chronic health condition and whose primary care provider is located in any of the following service areas. Eligible participants must also meet certain financial criteria.

Service area includes:

  • Farmington
  • Barrington
  • Gonic
  • Middleton
  • Milton
  • New Durham
  • Rochester
  • Somersworth
  • Sanbornville
  • Strafford
  • Berwick and Lebanon, Maine

Once a patient applies, a member of the MAP program reviews the application. A program representative will determine eligibility and submit applications on a patient's behalf to the primary care provider. Signed and returned applications will be sent to those pharmaceutical companies that offer free prescription programs for review.

Each individual pharmaceutical company sets final approval for participation in a prescription assistance program. If your application is accepted, medication will be sent to your primary care provider's office on your behalf, a process that can take six to eight weeks from the time the application is submitted.

Please contact the MAP office for additional information or application for the program at (603) 332-3100 x 8628. You must make an appointment. Walk-in patients are not accepted.

Advanced Directives
An Advance Directive is your written wishes and instructions for your health care if you cannot speak for yourself. For more information on Advance Directives and to download an advance directives form, please visit the Foundation for Healthy Communities.
