Medication Assistance Program

Our Medication Assistance Program (MAP) offers free prescription drugs to those who have a chronic health condition and whose primary care provider is part of the Frisbie Memorial Hospital Physician Practice Services.  Patients must also meet certain financial criteria to be eligible for the Medication Assistance Program. General Guidelines

Eligibility is based on Primary Care Physician, household income and liquid assets. Total liquid assets must not exceed $50,000.

  1. Applicant must be a legal US resident.
  2. Applicant must not have any outside assistance in paying for their prescription drug including: Veterans Administration Benefits; Private Insurance; or Medicaid (open case)

MAP office hours are Monday – Thursday, from 9am to 2pm. For general inquiries or to make an appointment, please call (603) 332-3100 ext. 8628 or (603) 330-7900 ext. 8628. We will review the patient’s application to determine eligibility. Qualifying applicants’ information will be submitted to their primary care provider for signature. Signed applications will then be forwarded to those pharmaceutical companies that offer free prescription programs for final review.
