How to obtain images
Providers who work at the hospital, or at any other of our off-site locations, have immediate electronic access to your images through our secure network if they are involved in your care. Copies of your exams can be made available to you on CD or DVD.
Advance notice of at least 24 hours is requested for image copies. Please call (603) 335-8400 and provide your name, date of birth, and the study being requested. When you arrive to pick up your images, you will be asked to sign a form authorizing us to release your images. To protect your privacy, you will also need to provide a photo ID, so please be sure to bring one.
Images can be picked up at the Radiology front desk Monday - Friday from 7:30 a.m. to 4 p.m. If you have called at least 24 hours in advance, they may also be picked up from x-ray staff evenings and weekends, though you may have to wait if staff are busy with patient exams. Please ask the hospital greeter in the lower lobby to call or page the x-ray technologist.
If anyone else is picking up your images, please be sure that this person has a written authorization form signed by you. Forms are available at the Radiology front desk, and may be signed at the time you have your exam or you can download this form, Authorization to Release Protected Health Information, here.
If you would like us to send a copy of your exam to a provider or another facility, we will also need the authorization form from you indicating the study you would like sent and the recipient's name and address. You may mail the form, or fax an authorization to us at (603) 335-8416. Please allow sufficient time for us to mail the images to the recipient, generally 3-5 business days.
