Our providers treat acute and chronic illnesses by providing preventive care and health education for men, women, and children of all ages. Because family history and heredity factors provide significant information in determining a patient’s health status, having a single physician treat all members of a family can be beneficial. We offer complete health care services by appointment for the entire family, including:
- Annual physicals
- Childhood immunizations
- Referrals to other health care specialists
- Prescription refills
- Same day acute care visits often possible
- Licensed Social Worker (LICSW) onsite for established patients
Requesting Medical Records
You may request copies of your medical records from Frisbie’s Medical Records department. For your request to be processed, you will need to fill out the Authorization to Use & Disclose Health Information form. For instructions on how to request your medical records, click here.
Payment and Insurance
All major insurance carriers are accepted, including Medicare and Medicaid.
Requesting Prescription Refills
If you need a routine prescription refill, please contact your pharmacy directly and request a refill. Your pharmacy will contact our office on your behalf – no need to contact our office directly. For more information, please refer to the Prescription Refills document.
